Miami-based virtual care firm HealthSnap has secured $25 million in growth financing led by Eastward Capital Partners. The capital infusion is earmarked for scaling the company’s AI-powered Advanced Primary Care Management platform, which currently supports over 80,000 active patient programs across 200 health systems nationwide.

The fresh funding arrives as HealthSnap experiences a period of rapid expansion, marked by a 400% revenue increase since mid-2023. By integrating Remote Patient Monitoring and Chronic Care Management into a single EMR-connected interface, the company aims to optimize clinical workflows without expanding headcount. CEO Samson Magid emphasized that the platform is designed to amplify clinician capacity rather than replace human expertise, leveraging agentic AI to manage patient data, clinical summaries, and routine outreach.

HealthSnap’s technology currently processes two patient measurements every second, with data indicating a 97% reduction in alert frequency for enrolled patients by their first year. Beyond operational efficiency, the company has demonstrated measurable clinical impact, including significant reductions in systolic blood pressure and hospital readmission rates. With this new facility, the firm plans to accelerate product development and commercial deployment, further embedding AI agents into the standard of care for major health networks like Prisma Health and Ascension Health.
