Insilico Medicine Targets Cardiovascular Risk with AI-Designed Molecule

With roughly 20% of the global population suffering from elevated lipoprotein(a) levels, Insilico Medicine has nominated ISM0900 as a new preclinical candidate. The oral small molecule inhibitor, developed via the company's generative AI platform, aims to address a significant unmet need in managing atherosclerotic cardiovascular disease.

The nomination of ISM0900 marks the ninth such milestone for the Cambridge-based firm since the beginning of 2026. Designed using the proprietary Chemistry42 platform, the compound selectively blocks the assembly of Lp(a), a known driver of major adverse cardiovascular events. While the global dyslipidemia market is expected to reach $18.7 billion by 2032, no therapies specifically targeting Lp(a) reduction have yet reached the market, leaving a gap that Insilico intends to fill.

Preclinical data indicates that ISM0900 offers distinct advantages over existing clinical-stage candidates. In animal models, the molecule achieved an 83.9% reduction in Lp(a) levels, reaching peak efficacy by the fourth day of treatment—a rate significantly faster than comparable therapies. Feng Ren, the company's Co-CEO and Chief Scientific Officer, highlighted that the oral format is intended to improve patient convenience and treatment adherence compared to current alternatives.

Insilico maintains that its AI-driven approach drastically compresses development timelines. While conventional drug discovery cycles often span four years, the company typically reaches the preclinical nomination stage in 12 to 18 months. Since 2021, Insilico has nominated 33 such candidates, 13 of which have secured IND approval. Founder and CEO Alex Zhavoronkov noted that the platform’s ability to generate multiple molecular options at scale increases the probability of success in finding viable treatments for aging-related diseases.
