Phase 3 Trial Fails to Validate Sacituzumab-Pembrolizumab Combo in NSCLC

The combination of sacituzumab govitecan and pembrolizumab failed to reach statistical significance for progression-free survival in patients with previously untreated, PD-L1-high metastatic non-small cell lung cancer, according to results from the EVOKE-03/KEYNOTE-D46 trial presented at the 2026 World Conference on Lung Cancer in Seoul.

In the study of 620 participants, the combination therapy yielded a median progression-free survival of 11.8 months, compared to 7.7 months for those receiving pembrolizumab monotherapy. Despite this numerical advantage, the hazard ratio of 0.81 fell short of the prespecified threshold for statistical significance. Interim analysis for overall survival showed 21.5 months for the combination arm versus 22.8 months for the monotherapy group, indicating no significant benefit at this stage.

Giannis Mountzios of the Henry Dunant Hospital Center noted that while the experimental regimen achieved a higher objective response rate of 55.6%—against 43.7% for pembrolizumab alone—it did not satisfy the trial's primary endpoints. Safety data indicated that 55.7% of patients in the combination arm experienced Grade 3 or higher treatment-related adverse events, compared to 16.5% in the monotherapy group. Common side effects included anemia, alopecia, neutropenia, diarrhea, and nausea, consistent with the known safety profiles of the individual drugs.
