Zipalertinib Combination Boosts Survival in First-Line Lung Cancer Trial

Patients with advanced non-small cell lung cancer harboring EGFR exon 20 insertion mutations saw their progression-free survival extend by six months when treated with zipalertinib alongside standard platinum-pemetrexed chemotherapy, according to phase 3 REZILIENT 3 trial data presented at the 2026 World Conference on Lung Cancer.

The international study tracked 279 patients, revealing that the combination therapy reduced the risk of disease progression or death by 50% compared to chemotherapy alone. Blinded independent reviews recorded a median progression-free survival of 14.5 months for the treatment group, significantly outperforming the 8.5-month benchmark set by the control arm. Benefits remained consistent even among patients with brain metastases, where a hazard ratio of 0.38 was observed.

Professor Daniel Tan of the National Cancer Centre Singapore noted that the regimen also yielded a 65% objective response rate, compared to 40.3% for those receiving only chemotherapy. While Grade 3 or higher adverse events appeared in 87.1% of the combination group—largely due to hematologic factors—researchers reported no new safety signals. The trial, registered as NCT05973773, continues to monitor overall survival as the study matures.
