Two pharmaceutical giants, AbbVie and Galderma, effectively dictate the digital patient pipeline in the $25 billion aesthetic medicine industry. According to the 2026 AI Visibility Index, these companies secure 80% of all AI-generated citations, leaving thousands of independent medical spas to compete for a mere 5% of search visibility.

The report, issued by 5W AI Communications, analyzed how major AI engines like ChatGPT, Claude, and Google AI Overviews route potential patients to providers. The findings reveal a stark consolidation of market influence, where AbbVie/Allergan leads with 47% of citation share, followed by Galderma at 33%. Smaller competitors Merz Aesthetics and Revance manage to capture the remainder of the top-tier visibility.
This concentration of influence creates a significant "Visibility Gap" for the roughly 12,000 facilities operating nationwide. While individual brands like Botox maintain near-total dominance—scoring 95 out of 100—independent med spas and even large chains like LaserAway struggle to gain traction against pharmaceutical marketing power. The data underscores a shift in consumer behavior where AI-driven answers serve as the primary gatekeeper for medical consultations, often excluding local providers before a patient makes a single inquiry.
